CONTRACT

I. Fees & Tuition 

a. A first-time registration fee of $40 per dancer/ $55 per family is due upon registration. *Twinkle Star Dancers Registration is $59 (if a student returns to a TSD class the following year, the fee is the same, if they level up to other classes, then the other fees apply). Continuing Student Registration Fees are reduced to $20/student or $40/family and will be charged to the card on file annually from original registration date. (*A Continuing Student is one who returns within 12 months of the last registered class. If returning after 12 months, the student will be charged the first-time registration rate.) b. Your credit card (Visa, MC, AMEX, or Discover) is required upon registration. Recurring payments will be automatically charged on the 1st of each month in the amount applicable to the number of registered classes and current contract. If paying per class (drop-in classes), fees must be paid prior to the class or your card on file will be charged in the amount of $20. Automatic Payment for contracted tuition will continue until the student is officially dropped by the parent/guardian. To opt out of automatic payments, you may provide an alternate method of payment (alternate credit card or cash only) prior to the 1st of the month. A credit card on file is still required and will be charged in the event that tuition is not paid by the 1st of the month or after any unpaid classes are taken. We do not accept checks as payment. c. If your credit card declines, a $10 fee will be applied to your balance due. Until your balance due is resolved, students will not accumulate make-up classes or be permitted into their regularly scheduled classes. d. To cancel future charges, classes must be dropped in writing before the 1st of the month. Failure to do so will incur a regular monthly charge that is non-refundable. e. Tuition is non-refundable and non-transferable. 

II. Adding or Dropping Classes

a. To add or drop a class, the front desk must be notified in writing. Monthly tuition will continue to be applied until notification has been received. 

III. Absences & Make-ups

a. The studio must be notified if a student will be absent. In the event of an injury, if able, the student should attend class to observe. Make-ups are permitted only if your account is current and you have prepaid tuition. You have 30 days from the time you miss a class to use the earned make-up class. Make-ups are limited to absences due to illness only and may be done in a class of similar level that you are not registered for. Make-ups may not be used for classes from which they were earned. Check in at the front desk prior to the make-up class. 

IV. Viewing Classes

a. Parents and visitors are not permitted inside the classroom to ensure the dancers stay focused and have no distractions. Our studio lobby is open for viewing classes via the windows in the lobby or the CC monitors for the downstairs rooms.  

V. Dress Code

a. The following dress code is required in order to participate. i. Ballet classes require pink ballet shoes, pink ballet tights, and hair in a tight, neat bun. Ballet classes also require the following leotard: ii. Twinkle Star 18mo-6y: Pink Twinkle Star leotard/dress iii. Twinkle Star 4-5: Lavender leotard/dress iv. Twinkle Star 5-6: Blue leotard/dress v. 6-9yrs: Light Blue leotard vi. 10yrs-Teens: Black leotard vii. Boys: White T-shirt or Tank, any black dance pants and black or white ballet shoes viii. Tumbling, Jazz, and Tap classes require any type of form fitting clothing that allows your body to move with ease. For example: jazz pants, dance shorts, leotards, etc. As in other styles, street clothes are not appropriate, however, sweatpants, T-shirts & sneakers are permitted for Hip Hop. Check with the front desk on how/where to purchase the class dress code. b. Students not in appropriate dance attire/uniform may not be invited to dance; it is up to the discretion of the classroom teacher. 

VI. Staying Informed

Most communication is delivered via email or text (Phone #) on the client profile or and should be kept updated. All newsletters are posted in the lobby of the studio, and sent via email to clients during the first week of the month and also found on the website. A parent information meeting is held and recorded every 3rd Tuesday of the month via Zoom then emailed out within 2 days.  Client is responsible for all information sent and must notify the office staff if emails are not received (be sure to check your spam folder). Information is also posted on our social media sites: Facebook and Instagram.  

VII. Release of Liability and Assumption of Risk

a. I, the undersigned and/or being the legal and acting guardian representing a minor participant, hereby release, waive and discharge Studio M Dance Academy, its directors, officers, employees, agents, independent contractors and volunteers from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Studio M Dance Academy. I understand that dance activities as conducted and taught at Studio M Dance Academy have inherent risks of injury. I recognize that the participant is exposing themself to such risks when undertaking dance activities. I, the undersigned and/or being the legal and acting guardian representing a minor participant assume and accept all risks of injury or damages resulting from such dance activities.

VIII. Medical Emergency

a. I, the undersigned, give permission to Studio M Dance Academy, its directors, officers, employees, agents, independent contractors and volunteers to seek medical treatment for the participant in the event they are not able to reach a parent, guardian or emergency contact. I also agree that I will be responsible for any financial debt incurred by said action. I have declared on this form any physical/mental challenges, limitations, restrictions, or condition and/or declare the participant to be in good physical and mental health.

IX. Marketing Release

a. Images of the participant may be used in Studio M Dance Academy ads, promotional videos, website material, or various other marketing. These images will be used for Studio M Dance Academy purposes only, and will not be given or sold to outside companies. ⃞ Opt Out_________________
